Tumor cells highly express PD-L1, which binds T cell surface PD-1 receptors, sending T cells a 'shutdown' signal — causing T cells to develop immune tolerance to the tumor. PD-1/PD-L1 inhibitors block this signal, releasing the T cell 'brake' and allowing previously suppressed anti-tumor T cells to reactivate and resume attacking the tumor. They don't 'enhance' immunity — they 'remove tumor suppression of the immune system,' restoring the immune system's inherent capacity.

Cancer screening (colonoscopy, mammography, Pap smear) directly addresses the 'gaps' in immune surveillance: when surveillance lets an abnormal cell slip through and it begins forming an early tumor (like an adenoma or carcinoma in situ), at this stage the tumor is small, hasn't yet developed complete immune evasion capabilities. Screening detects it; surgical removal eliminates it before it further evolves into a more dangerous invasive cancer — medicine proactively compensating for immune surveillance limitations.

Indirect evidence supports this. Chronic stress through chronically elevated cortisol reduces NK cell activity (the first line of immune surveillance). Research shows chronically high-stress individuals have significantly lower NK cell activity than low-stress controls. Theoretically this reduces clearance capacity for early cancer cells. But the evidence chain from animal experiments to human epidemiology is still being refined; quantified impact cannot yet be definitively stated. Chronic stress's influence on cancer risk likely works through multiple simultaneous mechanisms: immune suppression, inflammation promotion, and behavioral changes.

Main reasons include: low tumor mutational burden (few neoantigens, T cells lack sufficient targets); completely absent MHC-I (T cells can't recognize any neoantigens); overly immunosuppressive tumor microenvironment (even releasing PD-1 brakes, T cells can't survive and function in the tumor microenvironment); and T cells too deeply exhausted (blocking PD-1 can't effectively restore function). This is also why researchers are exploring combinations of PD-1 inhibitors with other treatments (chemotherapy, radiation, targeted therapy, CAR-T).
