Multiple myeloma (MM) is a plasma cell malignancy where malignant plasma cell clones massively proliferate in bone marrow, occupying bone marrow space and disrupting normal hematopoiesis and immune function. These malignant plasma cells secrete large amounts of monoclonal abnormal immunoglobulin (M protein) with no normal protective function, while suppressing normal plasma cell production of protective antibodies, leading to recurrent infections. Main symptoms: bone pain (marrow destruction), anemia (normal hematopoiesis suppressed), kidney damage (M protein deposition), and recurrent infections (normal immunoglobulin reduced).

Primarily a viral mutation problem, not plasma cell function itself. Influenza virus undergoes significant mutation each year; last year's vaccine-produced long-lived plasma cells secrete antibodies that may have dramatically reduced recognition efficiency for this year's mutated new strains. Additionally, older adults' new plasma cell establishment efficiency declines and long-lived plasma cell niche space is reducing, meaning even with new vaccine administration, fewer protective long-lived plasma cells are built than in younger years — these two factors combined explain weakening flu vaccine protection in older adults.

Generally yes, but requiring physician individualized assessment. Autoimmune disease patients (like rheumatoid arthritis, SLE) typically have already impaired immune function, and many use immunosuppressants, placing them at higher infection risk than the general population — making vaccine protection especially important. But some attenuated live vaccines (like varicella vaccine, certain live flu vaccines) may carry risk in severe immunosuppression — physicians need to evaluate. Inactivated and protein subunit vaccines are generally safe and should be actively received.

Rapidly declining antibody levels usually mean primarily short-lived plasma cell-dominated early antibody responses without establishing sufficient long-lived plasma cells. This may occur in mild infections — mild illness may produce enough antibody response for recovery but insufficient stimulus intensity to drive complete germinal center reactions and long-lived plasma cell formation. By comparison, well-designed vaccines (with adjuvants) are often more effective than mild infection at driving long-lived plasma cell formation, producing more durable antibody levels.
