The Birth of CAR-T (From Failure to Breakthrough)
Retrofitting T cells into precision-guided weapons
编辑部精选 · 一分钟理解
CAR-T cell therapy — Chimeric Antigen Receptor T cell therapy — takes patients' own T cells from blood, genetically engineers them in the laboratory to express a specially designed receptor that can precisely recognize specific proteins on tumor cell surfaces, then massively expands these modified cells and reinfuses them into patients to hunt tumors.
In 2017, the first CAR-T product received FDA approval for childhood acute lymphoblastic leukemia.
That year, some patients previously told 'there are no treatment options' achieved complete remission.
This therapy's birth went through nearly thirty years of failure, abandonment, and restart.
核心要点
01
CAR's core concept was proposed by Eshhar in 1989 (antibody recognition domain + T cell signaling domain), but first-generation CAR failed in vivo due to missing co-stimulatory domain — until CD28/4-1BB was added in 2002 did it truly activate.
02
2012 Emily Whitehead case was CAR-T history's dual turning point: proved complete remission feasibility in childhood B-ALL, and established tocilizumab for CRS control as the clinical standard.
03
2017, first CAR-T product received FDA approval; currently mainly effective in blood system tumors (B-ALL, large B-cell lymphoma, multiple myeloma); solid tumors face tumor microenvironment suppression and target specificity challenges, still being tackled.
04
CAR-T's main limitations: individualized custom manufacturing (high cost, long cycle), CRS and neurotoxicity management requirements, limited solid tumor efficacy. Next-generation technologies (off-the-shelf allogeneic CAR-T, CAR-NK, armored CAR-T) are systematically attempting to address these problems.
05
CAR-T's success is accumulation of thirty years of countless failures — it proves: before multiple bottlenecks in basic science, engineering technology, and clinical translation are simultaneously broken through, even a correct idea can sleep for decades.
