Any factor that disrupts peripheral tolerance — declining Treg function, infection-triggered local inflammation providing 'co-stimulatory signals,' tissue damage exposing hidden self-antigens — can cause previously suppressed self-reactive T cells to 'wake up,' detonating autoimmunity. This also explains why infection is one of the most important triggers of autoimmune disease: infection creates an inflammatory environment where previously 'anergized' self-reactive T cells suddenly receive the co-stimulatory signals they'd always been missing, awakening from dormancy. Once awakened, even after the infection subsides, these cells' self-attacks are often difficult to call back.
4. How common is autoimmune disease — an underestimated chronic disease burden
Over eighty recognized autoimmune diseases exist globally, with a cumulative patient count exceeding eighty million — over twenty-four million in the United States alone. Female predominance is one of autoimmunity's most striking epidemiological features. Current explanations include: estrogen's enhancing effects on the immune system (making B cells and T cells more easily activated), more immune-related genes on the X chromosome (females have two X chromosomes, enabling more diverse expression), and pregnancy-related immune regulatory fluctuations.

Autoimmune disease isn't a terminal illness — the vast majority of patients can maintain near-normal quality of life under standardized treatment. It also isn't one that must inevitably progress to deterioration — extensive research proves that early standardized treatment can significantly slow organ damage progression, even allowing disease to maintain long-term remission.
Treatment adherence is the most important and most easily overlooked issue in autoimmune disease management. Research shows the most common reason autoimmune patients stop medication isn't side effects — it's 'feeling better.' Symptom relief causes people to mistakenly believe the disease has disappeared and medication can stop. The cost of this misunderstanding is often an acute relapse weeks later, plus potentially irreversible organ damage. Understanding that 'remission doesn't equal cure, and maintaining treatment is a necessary means of preventing relapse' is one of the most critical insights in long-term management.
Autoimmune patients also face a special challenge: how to communicate effectively with the healthcare system. Because symptoms are diverse and multiple systems are involved, patients often need to see multiple specialties simultaneously, and medical information easily fragments. Establishing a 'core physician' — typically a rheumatologist or immunologist — to coordinate treatment plans across specialties is the evidence-supported best management model.
