HIV specifically attacks CD4+ helper T cells (it enters cells via the CD4 molecule). CD4+ T cells are the commanders of the entire immune response — without them, CD8+ T cell killing efficiency falls, B cells can't produce high-quality antibodies, and regulatory immunity breaks down. When CD4+ cell counts drop below 200/μL, AIDS develops and opportunistic infections — pathogens a healthy immune system handles easily — begin to kill. HIV's lethality isn't direct killing; it destroys the immune command system.

The core problem is self-reactive T cells that should have been deleted during thymic negative selection escaped screening into blood circulation; or normal regulatory T cells (Treg) have weakened function and can no longer effectively suppress these self-reactive T cells. The result: T cells (and the self-antibodies produced by B cells they command) begin attacking the body's own tissue — joints (rheumatoid arthritis), skin and kidneys (lupus), pancreatic islets (type 1 diabetes).

T cell exhaustion is a state of functional impairment that T cells develop under chronic antigen stimulation — such as in chronic viral infections or the tumor microenvironment. T cells chronically in an activated state upregulate a series of inhibitory receptors (most famously PD-1), becoming functionally sluggish with reduced proliferative capacity and decreased cytokine secretion. The mechanism behind PD-1 checkpoint inhibitors in cancer immunotherapy is precisely reactivating these exhausted T cells so they resume attacking cancer cells.

Chemotherapy drugs (particularly cytotoxic agents) indiscriminately kill rapidly dividing cells. Cancer cells are the intended targets, but rapidly dividing hematopoietic progenitor cells in bone marrow (including T cell precursors) are also affected. The result is reduced white blood cell replenishment — including T cells — causing transient immune suppression. This is why infection risk rises during chemotherapy, requiring strict protective measures and close blood count monitoring.
