I. A 1973 experiment still being cited today
In 1973, Loma Linda University researcher Albert Sanchez conducted a study repeatedly cited since. He had healthy subjects ingest equal amounts (100g) of glucose, fructose, sucrose, honey, or orange juice separately, then drew blood to test neutrophil phagocytic capacity — their efficiency at engulfing and eliminating bacteria. Result: one to two hours after sugar intake, neutrophil phagocytic capacity dropped approximately forty to fifty percent. This effect persisted approximately five hours. The control group (fasting water) showed no change in phagocytic capacity. This study has methodological limitations (small sample size, in vitro testing), but the question and direction it raised has been repeatedly validated in subsequent research: acute blood glucose elevation does suppress immune cell function for a period.
One mechanism involves vitamin C. Leukocytes (including neutrophils) need high concentrations of vitamin C to function normally — intracellular vitamin C in leukocytes is ten to fifty times higher than plasma. Vitamin C transport uses the same membrane transporter proteins (GLUT) as glucose. When blood glucose rises sharply, large amounts of glucose occupy these transporters, competitively inhibiting vitamin C uptake. Result: leukocytes in high-glucose environments are actually in a state of functional vitamin C deficiency. So drinking large amounts of sweet beverages before a cold, or eating large amounts of sweets while ill — not just 'not helpful,' at the immune function level, may actually be making things worse.

2. AGEs: the 'caramelization' marks sugar leaves in your body
If you've ever grilled meat, you've seen the Maillard reaction — food under high heat, proteins and sugars react, producing the brown outer crust and distinctive aroma. A similar reaction also slowly occurs in your body, requiring no high temperature, just sufficiently high blood glucose concentration and enough time. The products of this process are called AGEs (Advanced Glycation End-products). When body glucose concentrations are chronically elevated, sugar molecules undergo non-enzymatic glycation reactions with proteins, producing AGEs. These molecules deposit in blood vessel walls, kidneys, retinas, and immune tissue, damaging these structures' normal function.
AGEs' immune impact mainly occurs through a signaling pathway called RAGE (receptor for AGEs). AGEs-RAGE binding activates NF-κB — chronic inflammation's most central transcription factor, driving continuous secretion of pro-inflammatory cytokines like TNF-α, IL-6, IL-1β. In other words, long-term high-sugar diets continuously deliver chronic inflammation signals throughout the body through the AGEs-RAGE-NF-κB pathway. This explains why type 2 diabetics' chronic inflammation levels are systematically elevated, and why high-sugar diets (even without reaching diabetes level) correlate with inflammation marker (CRP, IL-6) elevation.
3. Fructose: the more insidious danger
When discussing sugar and immunity, one sugar deserves special mention: fructose, especially industrially produced high-fructose corn syrup (HFCS, common in sugary beverages and processed foods). Fructose metabolism mainly occurs in the liver, unlike glucose which can be utilized throughout the body's tissues. Large amounts of fructose entering the liver: promotes de novo lipogenesis, increasing visceral fat accumulation (visceral fat is one of chronic inflammation's primary sources); produces uric acid, and high uric acid directly activates the NLRP3 inflammasome, triggering IL-1β and IL-18 secretion — the mechanism of gout, also a direct pathway by which fructose elevates systemic inflammation; and reacts with proteins faster than glucose, producing AGEs at more than ten times the rate of glucose.
High-fructose corn syrup has massively entered the food industry over the past few decades primarily because it's cheap, high-sweetness, and has good solubility. Sugary beverages, juice drinks, candy, cookies, and sauces — HFCS is ubiquitous. From an immune health perspective, HFCS is the most immunologically unfavorable category of refined sugars, worth actively identifying and avoiding on food labels.
4. Sugar and the gut: an accelerator of microbiome dysbiosis
Sugar's third destructive pathway through immunity operates through the gut. High-sugar diets (especially refined sugars and fructose) are opportunistic pathogens' favorite food. Short-chain fatty acid-producing beneficial bacteria (like Bifidobacterium, Lactobacillus, Akkermansia muciniphila) gradually decrease in high-sugar, low-fiber environments because they need dietary fiber as their substrate. Decreased microbiome diversity directly affects gut barrier integrity — short-chain fatty acids (especially butyrate) are gut epithelial cells' energy source; without them, gut epithelial cells' tight junction proteins decrease, barrier permeability increases. Bacterial endotoxins (LPS) more easily enter blood, triggering metabolic endotoxemia, elevating systemic inflammation.
A phenomenon you can observe in yourself: people with long-term high-sugar diets often have gut discomfort (bloating, alternating constipation and diarrhea) — this is a signal of microbiome dysbiosis and simultaneously a signal that the gut-immune axis is dysregulating. Good news: gut microbiome responds to dietary changes quite quickly — research shows that reducing refined sugar and increasing dietary fiber, measurable improvements in microbiome composition can be seen within three to seven days. This pathway goes in both directions.
5.Sugar and aging: AGEs accelerate immune cell aging
Glycation reactions don't only occur in blood vessels and connective tissue — immune cells' proteins are also glycated. Immune cell surface receptors (like T cell receptors, NK cell activating receptors) are reduced in functional efficiency when glycated; intracellular enzymes, when glycated, affect metabolic efficiency and signal transduction. This mechanism makes high-sugar diets an accelerating factor for immune aging — not just indirectly accelerating aging by elevating inflammation, but directly damaging immune cells' molecular mechanisms, making them age faster. For those over forty, this deserves particular attention: the immune system is already declining due to age, and every accelerating factor shortens the 'window when the immune system is still working efficiently.' High refined sugar intake is one of the controllable accelerators.
6. Not eliminating sugar, but replacing it — practically actionable advice
This article isn't saying you can't touch a single gram of sugar. Extreme dietary restriction isn't sustainable for most people, and stress itself damages immunity. Practically actionable directions: primary goal: dramatically reduce liquid sugar — sugary beverages (bubble tea, juice drinks, carbonated beverages, sports drinks) are the single largest source of refined sugar intake for modern people. Reducing one sugary beverage per day is the single most direct dietary change affecting immunity. Read food labels and identify 'sugar aliases': high-fructose corn syrup, maltodextrin, glucose syrup, sucrose, white sugar, palm sugar, brown rice syrup — all are different forms of refined sugar.
Replace with natural sweets: fresh fruit (contains fiber, blood glucose response much slower than juice and sugar), small amounts of dark chocolate (cocoa content 70%+, high polyphenol content, some anti-inflammatory effect). Increase dietary fiber intake — paired with reducing sugar: fiber not only slows sugar absorption but directly feeds beneficial bacteria, simultaneously protecting the gut-immune axis from two directions. WHO's recommendation: free sugars (refined and added sugars) intake should be below ten percent of daily total calorie intake (approximately fifty grams), ideally below five percent (approximately twenty-five grams). One 500 mL cup of bubble tea typically contains fifty to seventy grams of sugar, already exceeding the daily limit.
