My anti-CCP antibody is positive but joints don't hurt yet — do I need treatment?

This is a frontier question in rheumatology. Positive anti-CCP without symptoms is called 'pre-clinical rheumatoid arthritis' (Pre-RA). Approximately thirty to forty percent of such individuals will develop true RA within five to ten years. Current evidence doesn't support starting conventional DMARDs in the asymptomatic stage, but strongly recommends: quitting smoking, treating periodontal disease, regular follow-up (every six to twelve months rechecking inflammatory markers and joint status). Once joint swelling or elevated inflammatory markers appear, seek immediate care and start treatment.

Is long-term methotrexate safe?

At standard RA doses (usually ten to twenty-five mg weekly) with proper monitoring, methotrexate's long-term safety is supported by decades of clinical evidence. Main side effects (liver function abnormalities, bone marrow suppression) under standardized follow-up (blood count and liver function every one to three months) can generally be detected early and managed. Simultaneously supplementing folic acid (5mg/week, taken on a different day than methotrexate) can significantly reduce side effects. Methotrexate's clinical safety record is far better than most patients fear.

My fingers are already visibly deformed — is treatment still worthwhile?

Yes, very much so. Existing joint deformity indeed can't be reversed, but standardized treatment can: prevent further joint destruction and worsening deformity; control systemic inflammation and reduce cardiovascular and pulmonary disease risk; significantly improve pain and function, raising quality of life. 'Stopping further deterioration' is itself an important treatment goal. Many late-stage patients see significant quality of life improvement after beginning standardized treatment.

Can RA patients exercise normally?

Not only can they, they should. Regular low-to-moderate intensity exercise is an important component of RA management. Swimming and water aerobics have the lowest joint load with excellent effects; walking, yoga, and tai chi are also good choices. Exercise helps improve joint range of motion, strengthen muscles (reducing joint load), reduce fatigue, and improve psychological state. During acute swollen/painful joint phases (disease active period), exercise intensity should be appropriately reduced — but shouldn't completely stop.
