Osteoarthritis (OA) is mechanical wear and degeneration of joint cartilage: predominant in those over fifty, asymmetric, mainly weight-bearing joints, morning stiffness usually under thirty minutes, no systemic symptoms, treatment direction is pain management + physical therapy + joint replacement. These two are fundamentally different diseases with completely different treatment directions. Treating RA as OA misses the most critical treatment window.
2. Why does the immune system attack joint synovium — the secret of protein citrullination
The core immunological puzzle of rheumatoid arthritis is: why does the immune system treat joint synovium as the enemy? The answer points to 'protein citrullination.' In smokers' lung tissue, periodontal infection gum tissue, and the inflammatory environment of joint synovium, PAD enzymes (peptidylarginine deiminases) are massively activated, converting arginine to citrulline in proteins, changing protein structure.
For people with specific HLA-DR4 genetic backgrounds, the immune system identifies 'citrullinated proteins' as 'foreign threats,' producing anti-citrullinated protein antibodies (ACPA — also the clinically measured 'anti-CCP antibody'). These antibodies can be detected in blood five to ten years before RA symptoms appear — meaning disease initiation begins far earlier than when joint pain is perceived.
