I have long-term insomnia — does that mean my immune system is definitely poor?
Not necessarily 'definitely poor,' but it does face higher risk of functional impairment. Chronic insomnia patients' immune function at the group level is indeed lower than normal sleepers — NK cell activity is lower, inflammatory markers are higher, vaccine response efficacy is reduced. But individual variation is large. Importantly: insomnia is treatable. Cognitive behavioral therapy for insomnia (CBT-I) is currently the highest-evidence treatment for chronic insomnia, more effective than sleep medication and without dependency risk. If you have chronic insomnia, seeking CBT-I treatment has more practical value than worrying about 'poor immune system.'
Can weekend catch-up sleep compensate for workday sleep deficits?
Some function can recover, but not completely. Daytime drowsiness basically recovers after catch-up sleep, but metabolic function and inflammatory markers haven't fully returned to normal. The best strategy remains maintaining consistent sleep duration across workdays and weekends (variation no more than one hour). Using 'catching up on weekends' to justify long-term sleep deprivation is biologically unsupported.
Do melatonin supplements help immunity?
Some evidence, but requires correct use. Melatonin isn't a 'sleep drug' but a 'rhythm signal' — its function is adjusting sleep phase, not directly inducing sleep. Evidence supports use in jet lag, shift workers, and older adults with reduced endogenous melatonin. For dosage, low doses of 0.5 to 1 mg are as effective as commonly used 3 to 5 mg with fewer side effects. Use under physician guidance rather than self-medicating at high doses.
Seven hours or eight hours — which is better for immunity?
The spleen is the core organ for immune filtration of blood, especially critical for clearing encapsulated bacteria (Streptococcus pneumoniae, Haemophilus influenzae, Neisseria meningitidis). After splenectomy, clearance capacity for these bacteria drops dramatically, and the risk of Overwhelming Post-Splenectomy Infection (OPSI) rises significantly — with high mortality when it occurs. This is why splenectomy patients need lifelong vaccination against these bacteria and need to seek medical care and antibiotics promptly at the first sign of infection.
